Care Partner / Case Manager

Visa sponsorship

Community Services · Sydney, NSW, Australia · Full-Time · $80,000 - $95,000/yr · Visa: Employer sponsored

Here is the role in brief. A Sydney aged care provider needs a Care Partner / Case Manager to organise home care for older Australians. The provider delivers person-centred care through the Support at Home program and wants someone compassionate and experienced for its close-knit team.

The work follows a clear pattern. First, you get to know each client and assess what they need. Next, you set up and manage an individual support plan. Then you coordinate the services and keep them running on time. You do this independently, as the team has a flat structure. Visa sponsorship is also available for the right candidates.

Step by step, build an English CV that lists your aged care roles, your assessment work and your experience with care plans. Organise your CV for this role with Build AI CV.
